Wraparound Oregon’s child and family teams provide the foundation for the development and execution of individual plans of care. Each team is formed around the strengths and needs of children and their families. Family members determine who should be the team and guide the decision-making to best fit the needs of their children. When children are referred into Wraparound Oregon, families are visited by the facilitator and family partner. They learn about wraparound and how the process can be helpful to their children. Families agree to participate and then they begin to identify who should be on their child and family team.
